Following verification of an offender's DC qualifying offense, the offender is given a DNA Sample Collection Referral Form which he/she signs in the presence of their Community Supervision Offficer (CSO). The offender is then scheduled for DNA collection.

Prior to DNA collection, the offender's identity will be positively confirmed. If accompanied by their CSO, the CSO will give verbal confirmation of the offender's identity. The offender's date of birth, Social Security number, and FBI identification number will also be verified. Upon confirmation of the offender's identity, a trained CSOSA staff person will obtain a small blood sample from the offender.

After obtaining the sample, the Community Supervision Assistant will sign FBI form FD-936 and imprint the offender's index fingerprints on the form. The form is then signed by the individual who obtained the blood sample (sample collector). The sample collector will then package the initialed vial containing the blood sample along with the completed form FD-936 and affix the evidence seal. The collection kit for each offender must be completed, sealed and set aside for mailing to the FBI before DNA collection can begin on another offender. CSOSA has initiated a state of the art chain of custody procedure to ensure integrity in the DNA collection process.
